Privacy Notice

The short version

The only information we hold about you is what you type into the form on this site: your name, your email address, the reason you picked from the list, and your message. We use it to reply to you and to keep you updated about Fortiva. We do not sell it, we do not rent it, and we do not trade it. If you choose to share your story with us, that form asks for a little more, and what we may do with it is your decision: nothing you send is published without us agreeing it with you first. This site sets no cookies and runs no analytics or advertising trackers. Ask us to delete your details and we will.

Who we are

Fortiva is a founding team preparing to launch two organisations: Fortiva Medical, which will develop medical products, and The Fortiva Foundation, which will focus on access, experiences and moments for people living with cancer. Neither is incorporated at the date of this notice. Until they are, the founders named on this website are responsible for the information collected here.

What we collect, and what we do not

We collect what you choose to give us:

• Your name
• Your email address
• The reason you selected for contacting us
• Anything you write in the message box

That is the whole list. There is no account to create, nothing to buy, and no payment details anywhere on this site. We do not ask for your date of birth, your address, your phone number or any government identifier. We do not build a profile of you, and we do not combine what you tell us with information bought from anyone else.

If you share your story

The story form asks for your name, your email, optionally where you are, whose story it is, and whatever you choose to write. It also asks the question we treat as the important one: what we may do with it. The default, already selected, is that we do nothing with it beyond reading it.

We will not publish it, quote it, or use any part of it in any material without coming back to you first and agreeing what goes out. That holds whichever permission you chose. Your selection tells us where to start that conversation, not where to finish it.

If you are writing about someone else, please send only what they are happy for you to send. If you are writing about someone you have lost, we will treat it with the care that deserves. Ask us to delete it at any time and we will, and we will tell you when it is done.

Health information, and what not to send

Fortiva is not a healthcare provider, and nothing on this site is a clinical channel. The forms are not monitored around the clock and they are not secure medical records. Please do not send us your diagnosis, your treatment plan, your test results, your medical record numbers or anyone else's clinical details. If you are in treatment now and want to know what scalp cooling options exist today, the right conversation is with your oncology team.

The story form is the one place you may want to write about your health, and that is fine. Telling us you had chemotherapy and what that was like is a personal account, and it is what we are asking for. A copy of your pathology report is not. If you do send us clinical information anyway, we will treat it carefully and delete it on request, but we would rather you did not have to.

Why we use it

To reply to you, and to send you occasional updates about Fortiva if that is what you asked for. Nothing else. We rely on your consent, given when you submit the form, and you can withdraw it at any time by emailing us. Withdrawing consent does not affect anything we did with your information before you withdrew it.

Cookies and tracking

This site sets no cookies of its own. There is no analytics package, no advertising pixel, no session tracking and no consent banner, because there is nothing to consent to.

Who else is involved

Three things, and that is the whole list.

Formspree carries both forms, the contact form and the story form, as two separate forms so the two do not mix. When you press send, what you typed passes through their systems on the way to our inbox. They are based in the United States.

The talk on this site is a YouTube video, and it plays here rather than sending you anywhere. It does not load at all until you press play, so nothing reaches YouTube before then. Once it is playing, YouTube receives the ordinary technical information any video player receives, and it runs in privacy-enhanced mode, which does not set viewing cookies for advertising.

Our web host keeps standard server logs of IP addresses and pages requested, for security and troubleshooting rather than to identify anyone, and our two typefaces are delivered by Google Fonts, which means Google sees your IP address when your browser fetches them. Neither involves your name or your email.

Some of these operate outside Canada, mainly in the United States, so information may be stored or processed there.

How long we keep it

Enquiries stay in our inbox for as long as the conversation is useful, and we review them periodically. If you asked for updates, we keep your name and email until you tell us to stop. Stories are held separately from enquiries, and we keep yours for as long as you are happy for us to. Ask us to delete anything and we will do it, normally within 30 days.

Your choices

You can ask us to:

• Tell you what information about you we hold
• Correct anything that is wrong
• Delete what we hold
• Stop sending you updates
• Send you a copy of what you gave us

Email info@fortivamed.com and say which one. We may ask a question or two to confirm it is really you, and then we will act on it. There is no charge. If you think we have handled your information badly, tell us first and give us a chance to fix it. You can also complain to the privacy regulator where you live. In Canada that is the Office of the Privacy Commissioner of Canada.

Children

This site is meant for adults. We do not knowingly collect information from children. If you believe a child has sent us something, write to us and we will remove it.

Keeping it safe

The site is served over HTTPS, so what you type is encrypted in transit. Access to the inbox is limited to the founders who need it. No system is perfect, and we will not pretend otherwise, but there is very little held here to lose.

Changes

We will update this notice as Fortiva takes shape, in particular once the two organisations are incorporated. The date at the top always tells you which version you are reading.

Fortiva Medical and The Fortiva Foundation are separate organisations. Neither is incorporated at the date of this notice, and The Fortiva Foundation is not a registered charity, so nothing given to it is tax deductible. Nothing on this website is medical advice, and no claim is made about how well any product works.
